Custom PDF forms can be prepared in Adobe Acrobat and attached to workflows as completed, non-editable documents.
This method works as a workaround for creating custom forms inside STACK | Build & Operate.
PDFs must be flattened (non-editable) before uploading to avoid form-field errors.
This workflow configuration supports teams that rely on standardized checklists or structured forms.
Prepare a Custom Form
To streamline your workflows, you can prepare a custom PDF form that can be uploaded as a finalized, non-editable document.
Create a workflow named Safety Checklists.

STACK Field AppNavigate to find the Safety Checklist template PDF.

STACK Field AppOpen the Safety Checklist PDF template in Adobe Acrobat.
Complete all required fields within the checklist.
When the form is finalized, click the three dots (more options) menu and select Print.
Select the Share icon and choose Save to Files to generate a flattened, non-editable PDF.
Click Save.
Attach the Completed Form to a Workflow
Once your form is finalized, you can attach it to a workflow to ensure it’s included in the steps your team follows.
Create a new Safety Checklist workflow.
Select Attach Files.
Tap +ADD.
Navigate to the Safety Checklist PDF.
Tap Open.
Close the window by tapping the X.
The completed form is now attached to the Safety Checklists workflow and available for reference within the process.
